報名方法 Enrollment Method：

參加者可於 A) 各大會夥伴機構索取並填妥報名表格；
                 B) 大會網站下載並填妥報名表格；(大會網站: www.hkyogathon.com)
                 C) 大會網站填妥網上報名表格；(大會網站: www.hkyogathon.com)

並支付港幣二百元作活動行政費。大會亦鼓勵參加者積極邀請親友捐款予大會受惠機構「香港乳癌基金會」。憑藉更多的捐助，可為更多罹患乳癌或受乳癌威脅的人士帶來希望！

Participants can A) obtain the enrollment form in our partnering organizations;
                        B) download and fill in the enrollment form; (Website: www.hkyogathon.com)
                        C) do the enrollment online; (Website: www.hkyogathon.com)

You will need to contribute HK$200 as the event administration fee. Participants are also encouraged to raise additional funds from friends and family to our beneficiary organization Hong Kong Breast Cancer Foundation (HKBCF). Your generous support and donations will make a positive difference in the lives of those living with or at risk of breast cancer.

繳交款項方法 Payment Method:

1) 於2012年10月22日前將活動行政費及額外籌得之善款以支票或銀行入數方式，連同填妥及簽署之報名表格以郵寄方式交予「香港瑜珈馬拉松」辦事處 (支票或銀行存款收據背面須寫上參加者姓名)；

2) 網上報名人士只需於2012年10月22日前，於大會網頁報名後以VISA 或 MasterCard 繳款。

*扣除行政費，所有籌得之善款將全數捐贈予「香港乳癌基金會」作慈善用途。

參加者需最少支付港幣二百元活動行政費用。請留報名表格及存款收據副本以便核對。因任何理由未能出席活動者將不獲退款。
大會歡迎參加者向親友籌募款項，捐款港幣一百元或以上方可獲發「香港乳癌基金會」發出的捐款收據(行政費用將不獲發收據)作扣稅之用；籌得港幣五百元或以上者更可獲發「瑜珈善心大使」獎狀。捐款收據將由參加者代為分發，多謝您的支持。
合資格參加者將大約於活動舉行前一個星期收到大會發出的確認通知書，詳列活動安排。如閣下於2012年10月24日仍未收到確認通知書，請致電3421 0807與「香港瑜珈馬拉松」辦事處聯絡。

1) Please post your administration fee and the extra sponsorship by cheque or bank-in slip, with the filled and signed enrollment form to the “Hong Kong Yogathon” Office on or before 22 October 2012 (please state your name at back of cheque or bank-in slip);

2) Pay by VISA or MasterCard after online registration on or before 22 October 2012.

*All extra sponsorship received will be donated to HKBCF in full, after deducting the administration fee;

Participants must contribute an administrative fee of HK$200 or more. Please keep a copy of enrollment form and bank pay-in-slip for verification. No refunds for non-attendance.
Participants are encouraged to raise funds from friends and families. HKBCF will issue a receipt to donation of HK$100 or above which is tax deductible (Receipt will not be issued for the administration fee). Participants who have raised more than HK$500 will receive a “Yogathon Ambassador” Certificate. The participant is also required to distribute the receipts to his/her sponsors. Thank you for your kind support.
Qualified participants will receive a confirmation letter with event details about 1 week before the event day. Please call the “Hong Kong Yogathon” Office at 3421 0807 if you do not receive the confirmation letter by 24 October 2012.
